Two days ago this newspaper told you that the cyclosporiasis outbreak sweeping our corner of two states had, for all its size, killed no one. That is no longer true. On Monday, Michigan's Department of Health and Human Services reported the outbreak's first two deaths — both people who, the department said, had "significant underlying health conditions that may have been impacted by cyclosporiasis and dehydration." No names, ages or hometowns were released. Michigan's confirmed case count has climbed to 11,234 as of Monday, with 193 hospitalizations as of July 30. MDHHS was careful, and correct, to add context: "Cyclosporiasis is generally not a life-threatening illness. Death resulting from cyclosporiasis is uncommon in the United States." Both things are true at once. It is rarely fatal, and it has now been fatal twice. Here is why it is our story and not merely a Michigan one. On the Ohio side, the state health department's most recent figure put Ohio at 4,013 confirmed cases, with Lucas County the highest in the state at 554 and Wood County at 446. Set that state figure beside the number Lucas County's own health department was posting on its dashboard — 1,207 — and the gap we flagged on Saturday is still open and still unexplained. The Blade reported that Lucas County added 11 cases over the weekend, which tells you the direction without settling the total. Whichever number you trust, the shape holds: northwest Ohio has been carrying the largest share of Ohio's outbreak, and the two states that share this lake share the worst of it. The federal picture remains stranger and smaller. The FDA's own multistate page still counts 1,947 illnesses across nine states, 98 hospitalizations, and zero deaths, last updated July 24 — a figure that lags because it counts only lab-confirmed cases tied to the traceback investigation. The suspected food is the same one we named Saturday: iceberg lettuce grown in central Mexico, subject of a July 17 Taylor Farms recall of Marketside-brand salad and shredded lettuce sold at Walmart. The honest caveat also holds: a Taylor Farms sample that first tested positive was later confirmed a false positive, and Ohio's health director said as recently as early July that no single common source had been confirmed. The lettuce link is strong. It is not the last word. And Cyclospora has a signature cruelty worth repeating — it takes 2 to 14 days to appear, seems to clear, and comes back. The treatment is a specific sulfa antibiotic, which is exactly why a diagnosis is worth getting rather than waiting out. El panorama federal sigue siendo más extraño y más pequeño. Buy whole heads instead of pre-washed bags, discard the outer layers, wash the inner leaves, and cook greens to 158°F where the dish allows it. If you are older or immune-compromised — the group both Michigan deaths came from — treat that advice as a floor, not a suggestion, and see a doctor early if watery diarrhea sets in and won't quit. In the summer of 2018, a cyclospora outbreak traced to salads served at McDonald's — supplied by Fresh Express — sickened hundreds of people across the Midwest, Ohio and Michigan among the states hit, and a separate outbreak that same season was tied to Del Monte vegetable trays. Cyclospora is a warm-months visitor that rides in on fresh produce grown far away, and it returns the way the algae bloom returns: seasonally, through a supply chain that begins somewhere you will never see. What is different in 2026 is the scale — eleven thousand cases in one state — and, as of Monday, the cost. Este no es el primer verano de la región con este parásito.
